To ensure the quality of your care including a through evaluation and one-on-one treatment for your follow up, I am only in network with MEDICARE and currently considered an out of network provider with all other insurance companies for physical therapy services.
You can call your insurance company to find your out of network benefits for physical therapy. Upon request, a detailed super-bill can be provided at the end of your session, which you will then submit to your insurance company for direct reimbursement or be applied towards your deductibles.
Initial Consultation: Free of charge for a 15-20 minutes phone conversation or through email to be sure that physical therapy evaluation and treatment is appropriate for you. All calls should be made within business hours (8-6pm).
Initial Evaluation & Treatment: 70-90 minutes: including a thorough review of your history, evaluation of your body, assessment of your needs to establish an appropriate plan of care, follow up by a treatment session.
My philosophy as a physical therapist is quality over quantity when it comes to patient care.
Follow up Treatment at Clinic: 60-70 minutes: which can include different forms of manual therapy depending on what your body needs, tailored home exercise program, yoga/meditation techniques, etc. You can always email, text or call regarding your plan of care.
Home Physical Therapy Visit: if you can't make it to the clinic, I can also come to you.
